Do you have a passion for helping people take ownership of their wellbeing in a clinical setting? Our SEL Care Coordinator helps empower patients to manage their health condition through individual coaching and education.

Essential Duties and Responsibilities
1. Understand and utilize basic nursing theory and knowledge of clinical standards to provide care coordination to our patients while acting within the scope of practice.
2. Assess patient's diagnosis and provide patient education to empower them to manage their health condition.

3. Under general supervision, design and implement new programs.

4. Develop, lead, and coordinate improvements to existing programs.

5. Provide performance data analysis, manage program milestones, and document project activities.

6. Act as a back up to our nursing staff to provide first aid; and perform intake assessments, blood draws, vital signs, injections, and other relevant procedures to ensure that patient care is safe, timely, efficient, and effective.
7. Integrate all facets of our health care program to deliver high quality care and collaborate with our health care team to analyze conditions for improved clinical outcomes and deliver high quality care.
8. Document, process, and maintain medical records and reporting in a responsible, timely, and confidential way.

9. Follow and apply SEL Values, Principles of Operations, and World Class Manufacturing Principles.

Required Qualifications

Licensed as a Registered Nurse (RN) in good standing with the State of Washington and/or Idaho.

At least two years of clinical experience as an RN in a primary care, urgent care or emergency department setting.

Current CPR certification.

Excellent communication skills.

Strong writing, documentation, and speaking skills.

Ability to learn new skills and assume new responsibilities.

Ability to work cooperatively in a team environment.

Background check results satisfactory to SEL.

Negative drug test result(s).

Preferred Qualifications
Care coordination/case management experience
